Effective Jan 1, 2017 the following CPT codes will no longer be used.
97001 Physical Therapy Evaluation
97002 Physical Therapy Re-Evaluation
The 2017 CMS physician fee schedule has introduced 3 new CPT 
codes for physical therapy evaluation and 1 new code for reevaluation. They are:
97161 Physical Therapy Evaluation low-complexity
97162 Physical Therapy Evaluation moderate-complexity
97163 Physical Therapy Evaluation high-complexity
97164 Physical Therapy Re-Evaluation
There is no difference in payment values for 97161-3, as CMS did not yet adopt different payment values.
